Have a student athlete at St. John's? Print out the Athlete's Packet, sign all the forms and return to the School Office, Attn: Wendi Matz. All athletes are required to have a physical. Print out the Physical Form and have a Physician fill it out. Return to the office prior to the first practice.

No athlete will be allowed to participate without these forms completed.
